Abstract
The Centers for Disease Control and Prevention recommends vaccinating children for 14 communicable diseases in a 10-shot series by 24 months of age. State, national, and global childhood immunization rates remain low despite active vaccination efforts. The purpose of this project was to create and implement a customized electronic medical record (EMR) vaccine report and evaluate the impact on childhood immunization completion rates by 24 months of age. After 4 months of implementing the EMR vaccine report, total immunization completion rates significantly increased (P = .0002). Health care providers can use the EMR to create and implement customized vaccine reports to improve immunization rates.
